Safe Anaesthesia Liaison Group 'Stop Before You Block' Survey

At the request of the Healthcare Safety Investigation Branch, HSIB, SALG has developed a new draft standard operating procedure (SOP) to prevent wrong side blocks.

Before this is further tested or released, we are conducting this short survey and are asking for your help. We realise that within even any single trust, there may be more than one ‘Stop Before You Block’ policy, so our questions are addressed to you as an individual:

1. Which Trust are you employed at and what is your job role?

2. Please describe your trust wrong side block/ ‘Stop Before You Block’ policy or policies (or see Q4 below).

3. How many wrong side blocks have occurred in your trust (to your knowledge) under this policy and over approximately what timescale?

4. Please attach a copy of your trust’s SBYB policy or policies if available.

5. If possible, please attach an anonymised copy of any Root Cause Analyses or Serious Incident Investigations into SBYB incidents, if you are able/allowed to?

6. Would you like to review and comment on the SALG draft SOP for wrong side blocks? If so, we will send you the draft with a further questionnaire.

Data will be anonymised by person and collated by institution and used to help inform the process of developing and implementing the draft SOP to prevent wrong side blocks. By choosing to partake in this survey, you give your consent for your data to be stored and processed in this way.
